Why site rotation matters

Repeatedly injecting into the same spot stresses tissue. Over time this leads to irritation, inflammation, lumps, poor absorption, and higher infection risk.

Site rotation isn’t an optimisation tactic—it’s basic injection hygiene.

What proper rotation prevents

  • Persistent soreness and tenderness
  • Lumps, nodules, and scar tissue

Most “bad injection sites” are overused sites.

How site rotation actually works

Rotation does not mean “random.” It means using a predictable pattern that gives each site time to recover before reuse.

  • Rotate between left/right sides
  • Rotate between different anatomical areas
  • Avoid injecting into the same exact spot repeatedly

If you inject frequently, rotation becomes more important—not less.

Simple rotation patterns (examples)

SubQ users

  • Abdomen: left → right → different quadrant
  • Alternate with thigh or flank if volume/frequency is high

IM users

  • Glute: left → right
  • Ventroglute or thigh as secondary sites
Rule

If a site is still sore, it’s not ready—pick another.

When not to inject into a site

  • Redness, warmth, or swelling
  • Hard lumps or thickened tissue
  • Bruising or visible irritation
  • Any sign of infection or skin damage

Rotation + hygiene = safety

Rotation reduces stress on tissue, but hygiene prevents infection. You need both.

  • Swab skin and vial tops every time
  • Never inject through irritated skin
  • Use new needles and syringes for every injection
  • Dispose of sharps immediately
